Registration Checklist – 2008-2009 School Year

First tuition installment has been submitted and received by The Learning Academy of College Park. Please make checks payable to The Learning Academy of College Park for your child’s/children’s registration fee(s), if applicable.
Registration Fees for each child must accompany registration forms for placement to be guaranteed.
Sign and return Tuition Agreement.
Complete the Registration Form in its entirety.
Thoroughly read The Parent Handbook and familiarize yourself with the policies & procedures of the school. (This will be given to you upon completion of your packet.)
Complete the Additional Student Information Form.
Complete The School as a Community Form.
Students enrolling into Kindergarten must complete the Testing Appointment Form.
New students enrolling into Kindergarten must complete a Previous School Form.
Children 6 months thru Pre-K must provide a current certification of immunization (Form 3231 only).
Students enrolling into Kindergarten must provide both an Immunization Document (Form 3231 only) and a Vision/Hearing/Dental Form.
New students enrolling into Kindergarten must provide a certified birth certificate.
